DEX SCORE Assessments are required for clients receiving service under any DEX funded program.
Please discuss with your organisation, the frequency and the percentage of your client base, that you are required to record SCORE data for. We have found this information differs between organisations, usually based around your Funding Managers interpretation of the guidelines.
Please follow the instructions below on how to create a SCORE Assessment in Ivo using the Manage Clients screen.
- Click on the Work tab at the top left corner of the Ivo window.
- Then click the Manage Clients button on the toolbar.
- Select the client name from the 'My Current Clients' list in the navigation panel on the left of the screen.
- Go to the 'DEX SCOREs' tab in the main part of the screen.
- Click on the 'Create new Assessment/SCORE' link to open the 'New DEX Assessment/SCORE' window.
- Click on the 'New File Note' link to create a new file note to attach the Assessment to. Alternatively, you can click on an existing file note from the list below.
- The 'New File Note' window will open, enter the details as normal.
- Select the Activity that the file note applies to, from the bottom section of the File Note window by clicking on it.
- Click 'Save and Close' at the bottom right corner to move to the next step.
- Select the File Note in the list.
- Click on the 'Continue' button in the bottom left of the window.
- The 'New Assessment' window will appear, from the 'Assessment By:' drop-down list, select 'SCORE Directly - client' from the list, as this indicates the client has answered the assessment questions for themselves.
- In the 'Circumstances' and 'Goals' domains, select one or two of the most appropriate statements that describe the clients current situation.
- Once completed, click the 'Save and Close' button at the bottom left to save the assessment record.
- You will return to the DEX Scores tab and the completed assessment will be listed by date.
- If you click on an assessment in the list, the answers will be displayed in the bottom section of the screen.
- By right clicking on the assessment in the list, you will be provided with options to 'Change to POST Assessment', or you can delete the assessment.
